Analysis: Debt crisis a big test for Speaker Boehner (Reuters)
WASHINGTON (Reuters) ? House of Representatives Speaker John Boehner looked over his restive Republican troops on Wednesday and told them: "Get your ass in line."
Boehner's backside may be on the line as well in a cliffhanger dispute over raising the debt ceiling by the rapidly approaching deadline of next Tuesday.
Boehner is pushing the House for a vote on Thursday for deficit-reduction legislation that his party hopes will emerge as the last best option for averting a debt default that could trigger economic calamity.
To get it passed, Boehner has to stare down rebellious Tea Party conservatives who feel the government has gotten too big and who under no circumstances want to raise the debt ceiling to permit more spending of borrowed money.
The politics are tricky because by getting elected last November on a wave of anti-government sentiment, the Tea Party activists helped propel Boehner into the House speakership, making him America's most powerful Republican.
This makes Boehner beholden to the Tea Party, but he also has to resist the take-no-prisoners approach they favor and actually get a workable option approved.
"Listen, it's time to do what is doable, and this bill isn't perfect," Boehner told conservative talk radio host Laura Ingraham.
Word from Capitol Hill was that Boehner and his backers were making progress in convincing some conservatives to go along with his plan, improving the chances of passage for legislation that calls for a two-stage increase in the debt limit coupled with spending cuts.
The argument he is using is that there are three choices: the Boehner option, a Democratic alternative in the Senate, or default.
The speaker is walking on a narrow beam. If he is seen as neglecting the demands of his right flank, he could leave himself open to being ousted as speaker, essentially making him the victim of a palace coup.
BOEHNER FACES DEFINING MOMENT
Some Tea Party members are already calling for his head.
"I think Boehner should step down. He's ineffectual," said Dale Chiusano, a retired federal employee who was among 40 people at a Tea Party rally on Capitol Hill on Wednesday.
If Boehner is unable to steer the House toward a package that will avert default, he would be blamed by Democrats, whether fairly or not, for sending the country into a crisis and giving them ammunition to use against Republicans in the 2012 elections.
Boehner may come out a winner among Republicans if his package is approved and the Democratic-controlled Senate makes it a key part of a compromise agreement that stops the runaway train toward default.
"This is his moment," said University of Virginia political science professor Larry Sabato. "One way or the other, his speakership is likely to be defined by what he does and what he can accomplish. Just as you can have a failed presidency, you can have a failed speakership."
If the Senate were to approve a reworked version of Boehner's plan, then he would have to get it approved again by his membership with whatever modifications the Senate made.
"I think this is still a heavy lift for him," said Norm Ornstein, a Congress-watcher at the American Enterprise Institute.
While Boehner is wrangling with his own Republicans, he also has run afoul of President Barack Obama.
As Washington's most powerful politicians, Obama and Boehner need to have a working relationship in order to take on the challenges ahead.
They famously played golf together earlier this summer but whatever goodwill they had built up was damaged last week when Obama expressed his irritation, saying he thought he had reached a deal with Boehner only to be left standing at the altar by him twice.
The two men then sparred on national television on Monday night, giving back-to-back speeches outlining their views on the debt dispute.
"I didn't sign up for going mano-a-mano with the President of the United States," Boehner was overheard to say afterward by CBS News.

16 dead, 10 missing in Ukraine mine blast (AFP)
KIEV (AFP) ? Sixteen miners died and another 10 were missing after a nighttime explosion in a coal mine in the Lugansk region in eastern Ukraine, the emergencies ministry said on Friday.
As a result of the early Friday accident, two more workers received burn injuries and were hospitalised, the ministry said in a statement.
"The fate of 10 miners remains unknown," said the Kiev-based emergencies ministry, adding that a total of 28 people worked at the mine when the explosion occurred at 1:57 am local time Friday.
A spokesman for the emergencies ministry's local branch in Luganks, speaking to AFP, cofirmed the number of dead.
He said however that three miners were hospitalised, while nine remained trapped in the coal mine. He could not say whether they were dead or alive.
Ukraine's energy minister Yury Boiko went to the scene of the accident at the Sukhodolskaya-Vostochnaya Coal Mine, part of the Krasnodonugol mining company, to personally take charge of the rescue operation.
Officials did not provide details on the cause of the blast. Most of the country's mine disasters have been caused by build-ups of methane gas.
Deadly accidents are frequent in the Ukrainian mines, most of which are located in the country's industrial eastern region. Many of the mines are underfunded and poorly equipped, while safety violations are rife.

Researchers uncover previously unknown pathway for advanced prostate
New research finds advanced forms of prostate cancer are not driven by testosterone as previously believed. The finding means targeting a newly discovered enzyme could improve prostate cancer outcomes for men whose disease has spread beyond the prostate gland.
UT Southwestern scientists found advanced cases of prostate cancer evade testosterone, previously thought to stimulate cancer growth.
The finding explains why testosterone deprivation therapy fails to eradicate prostate cancer tumors.
Dr. Nima Sharifi, assistant professor of internal medicine and senior author of the study in Proceedings of the National Academy of Sciences, explains testosterone converts to a more potent male hormone that fuels tumor growth when the disease becomes more advanced.
Dr. Sharifi says, ?The general assumption is that the tumor accelerates through testosterone when, in fact, the pathway goes around it to the most potent hormone. We both found the existence of this pathway in models and patients, and have shown that these resistant tumors are clearly driven by this other pathway.?
Understanding what drives prostate cancer means researchers can use different drugs that target the enzyme responsible for converting testosterone into a more potent hormone.
Sharifi explains different drug treatment for prostate cancer ??can be thought of as charting a map of the correct pathway. You have to figure out which way the river flows before you can block the river.?
The finding, according to Dr. Sharifi, will change the way prostate cancer is viewed. Treating advanced prostate cancer requires drugs targeting an enzyme that initiates hormone production earlier in the disease.
Testosterone is known to drive prostate cancer. The new finding shows advanced stages of the disease is driven by a different and more potent hormonal pathway that was previously unknown and bypasses testosterone altogether.

N.Korea proposes to resume 6-party talks unconditionally: report (Reuters)
SHANGHAI (Reuters) ? North Korea has proposed restarting stalled six-party talks on nuclear disarmament, without preconditions and as soon as possible, China's official Xinhua news agency said on Saturday.
Chinese Foreign Minister Yang Jiechi met with his North Korean counterpart Pak Ui Chun on the sidelines of a Southeast Asian security summit on the Indonesian island of Bali, it said.
During the bilateral meeting between the two countries, Pak said North Korea proposed restarting the talks unconditionally, Xinhua said, without elaborating.
Yang called the friendship China had with North Korea a "precious treasure," saying the two should continue to maintain high-level exchanges and deepen economic and trade cooperation.
South and North Korea held high-level talks for the first time in two years in Indonesia on Friday.
The surprise meeting between their nuclear envoys, which was described as cordial and lasted about two hours, was the first such contact between the two Koreas since the last round of the six-way nuclear disarmament talks in 2008.
South Korea, the United States and China, which hosts the six-way forum, have agreed on a three-stage process for envoys from Seoul, Pyongyang and Washington to meet first before the negotiations can resume. Japan and Russia make up the rest of the participants.
North Korea walked away from the six-way nuclear talks more than two years ago, but last year called for their resumption.

Hyderabad-- Gilead Sciences, Inc. (Nasdaq: GILD) announced today an expansion of its global access program in an effort to provide accelerated access to Gilead medicines for the treatment of HIV/AIDS. The changes announced today include new licensing terms with four India-based drug manufacturers - Hetero Drugs Ltd., Matrix Laboratories Ltd., Ranbaxy Laboratories Ltd. and Strides Arcolab Ltd. - for three drugs which are currently in late-stage clinical development. These Indian partners have played an active role in supplying treatment to patients in the developing world. In addition, Gilead is the first pharmaceutical company to enter a licensing agreement with the Medicines Patent Pool Foundation (the Pool).
The expanded licensing terms grant to the company's Indian partners and the Pool future rights to elvitegravir, an investigational integrase inhibitor; cobicistat, an investigational antiretroviral boosting agent; and the "Quad," which combines four Gilead HIV medicines in a once-daily, single-tablet regimen. Gilead licensed rights to commercialize elvitegravir from Japan Tobacco (JT), and JT is working in close partnership with Gilead to ensure future access to elvitegravir in the developing world.
"Gilead is proud to engage in innovative partnerships to expand access for patients in the developing world, and we welcome new opportunities to work with Indian manufacturers and the Medicines Patent Pool," said Gregg H. Alton, Gilead's executive vice president for corporate and medical affairs. "Our goal is to ensure that as new Gilead HIV therapies are developed and approved, low-cost versions will be rapidly accessible in developing countries without delay."
Licensing agreements are a cornerstone of Gilead's efforts to increase access to the company's therapies in the developing world. Since 2006, when Gilead initially established licensing agreements with Indian partners, the lowest available price of a Gilead antiretroviral has decreased to US$6.15 per patient per month, or 21 cents per day. Today, more than 1.1 million patients in developing countries receive Gilead HIV medication produced by Indian partners.
Gilead also makes branded versions of its HIV medicines available at significantly discounted prices (at the company's cost of manufacturing) in developing countries. A total of 1.6 million patients in developing countries are currently receiving either generic or branded Gilead HIV medicines - representing nearly one-fourth of the 6.6 million patients receiving HIV therapy in the developing world. Gilead's regional distribution partners help to manage pharmacovigilance and medical education efforts, and work to secure product registration in individual countries [for a full report on registration status visit www.gilead.com].
Medicines Patent Pool
Gilead has granted the Medicines Patent Pool similar licensing terms as its Indian partners. The Pool, established in July 2010 with the support of UNITAID, is working to partner with a number of pharmaceutical companies to expand global access to quality, low-cost antiretroviral therapy through the licensing of patents. Companies interested in producing generic versions of Gilead medicines for developing countries will be able to approach the Patent Pool to negotiate licensing terms.
"The Medicines Patent Pool is a creative new approach for increasing access to treatment by facilitating access to IP on essential medicines, and we appreciate Gilead's willingness to be engaged and involved with us from the beginning," said Ellen 't Hoen, executive director, Medicines Patent Pool Foundation. "We look forward to working together to expand access to much-needed antiretrovirals and fixed-dose combinations for patients in the developing world. Our expectation now is that other companies will follow Gilead's lead and join the Pool."
Global Access Licensing Terms
Gilead's original licensing agreements provided Indian manufacturers with non-exclusive rights to produce active pharmaceutical ingredient and finished product and sell generic versions of Gilead's HIV medicines Viread? (tenofovir disoproxil fumarate, or TDF) and Truvada? (emtricitabine/tenofovir disoproxil fumarate) in 95 developing countries, including India.
The expanded agreements include future rights to produce and sell generic versions of three Gilead HIV therapies, if and when they are approved. Elvitegravir, cobicistat and the Quad are investigational products and have not yet been determined safe or efficacious in humans. The agreements also allow the sale of Viread and Truvada in an additional 16 countries, and also allow Viread to be produced and sold for the treatment of chronic hepatitis B in the expanded territory.
Licensees will receive a complete technology transfer of the Gilead manufacturing process to support their efforts to obtain local regulatory approvals and scale up production as soon as possible following U.S. Food and Drug Administration approval of the pipeline products covered under the agreement.
"India's pharmaceutical industry has the scientific expertise and large-scale production capacity that are critical for addressing the challenges of the HIV epidemic - both in India and around the world," said Rajiv Malik, Director, Matrix Laboratories Ltd. and COO, Mylan Inc, which was one of the first partners to sign an expanded licensing agreement with Gilead. "We are pleased to expand our innovative collaboration with Gilead as we seek to help more individuals living with HIV in resource-limited countries."
Under the terms of the agreements, licensees are allowed to establish their own price and will pay a royalty on sales of finished product, which supports product registration, medical education and training, safety monitoring and other critical activities. In an effort to accelerate the development of pediatric formulations of HIV medicines and address growing treatment needs among children, Gilead will waive royalty payments on any pediatric formulations of Viread, Truvada or pipeline products that are successfully developed and brought to market.
Global Impact of HIV and Hepatitis B
According to the World Health Organization, significant progress has been made in recent years in increasing access to HIV treatment in the developing world. Between 2002 and 2010, the number of people in developing countries receiving antiretroviral therapy increased by more than 2,100 percent, from fewer
than 300,000 to 6.6 million. However, HIV treatment needs continue to grow, with as many as 15 million people requiring treatment now, and an estimated 30 million expected to require treatment within the next five years.
Chronic hepatitis B is a common and potentially fatal liver disease caused by the hepatitis B virus (HBV), which is up to 100 times more easily transmitted than HIV. Approximately 350 million people are estimated to be chronically infected with HBV worldwide.
Tenofovir disoproxil fumarate, the active ingredient of Viread, is a prodrug of tenofovir. Misrata youth goes from Playstation to front line (Reuters)
DAFNIYA, Libya (Reuters) ? When the war in Libya started, many young men now on the rebel front line at Misrata were so interested in computer games and mobile phones that older residents never thought they would turn into fighters. "Before the uprising, all those young men cared about was hair gel, clothes, music, mobile phones and hanging out in cafes," said Mahmoud Askutri, a businessman who has formed and funds the 1st battalion of the Al Marsa regiment, one of the rebel units fighting here to end Muammar Gaddafi's 41-year rule.
"But now they fight and are willing to die for a cause."
Amid the Arab Spring protests that swept the region early this year, the people of Misrata and elsewhere in Libya demanded greater freedom, so Gaddafi sent in the troops to silence their protests.
After those troops opened fire on demonstrators, the people of Misrata rose up, initially fighting back with petrol bombs and hunting rifles.
Since then, they have wrested control of Libya's third largest city from Gaddafi loyalists and, after mistakes that cost many lives, this army of former civilians has consolidated a front line 36 km (22 miles) west of Misrata.
They have recently encountered better trained troops and have moved forward slowly under sustained bombardment to conserve ammunition, hold territory and reduce casualties.
That they are around 10 km (six miles) east of Zlitan, the largest city between here and the capital, Tripoli, is testimony to the courage of the young men in this force.
"They treat me with great respect," Askutri said before a visit to the men of Al Masra on the front line. "But when I see them I do not feel worthy of that respect. A few months ago they were civilians. Now they are willing to die for their freedom."
Salah is typical of many young men on the front line here. The 20-year-old was attending medical school when the uprising started. Life was easy and he spent a lot of time playing soccer games on Playstation.
"Fifty fifty," he says of his record on Playstation.
Sitting with a group of other young men, he says he is a big fan of FC Barcelona. A second young man shakes his head and says he likes Real Madrid, while a third looks down at the Manchester United logos embossed on his shoes and says nothing.
Salah plans to return to university after the war, as he wants to become a cardiologist.
"But first we must beat Gaddafi," he says. "We cannot be free if we live under him."
"THIS IS MY GUN"
Mobile phones are common at the front line, even though the city has been without mobile reception since the uprising.
Young fighters use them to take pictures of each other and videos of battle. Some of them hand out email addresses, though again internet is available at very few spots in Misrata.
Another sign of the times is that the Al Marsa has an amateur videographer. Yezid, a slight 23-year-old microbiology student with round spectacles, carries a video camera to the front.
He has been wounded twice, with a bullet in his right thigh and a piece of shrapnel in his left knee that makes walking painful and running impossible.
"This is my gun," he said, holding up the camera with a smile.
Dressed in t-shirts, jeans and whatever sensible shoes they have, the teenagers and twentysomethings here have come a long way in just a few months. They joke when Gaddafi forces fire Grad rockets at them from nearby because they are not very effective at close range.
But like soldiers anywhere, what they do not like are mortar attacks where the bombs hit closer to the front line and cause more casualties.
"A Grad is no problem, but I don't like the mortars," said Ahmed, 21, an engineering student sharing a bunker with two friends who jokingly refer to it as a five-star hotel. "The small pieces of metal from the mortar cut you."
When the crump of a mortar is heard, many of the men in the line say "Allahu Akbar," or "God is Greatest," before it lands. Muslims believe that dying with those words on your lips brings you closer to God.
SHRAPNEL WOUND
Despite the concern about mortars, the men at the front all seem focused on their cause.
Asked what thoughts he had of a future beyond the war, one 21-year-old who gave his name as Ali shook his head.
"I don't care about that now," he said. "All I want to do is kill Gaddafi."
Since speaking to Reuters, Ali has sustained a shrapnel wound in the leg but has returned to the front line. Wounds for the "thowar" or revolutionaries have become, as Agila Erfaida, a lecturer at the University of Misrata's Faculty of Medical Technology puts it, "badges of honor."
Certainly the young men of a unit led by Tariq Madi, a former bank employee, are keen to show off their scars.
"Most of the men here have been wounded more than once," said Madi, 36, who managed the safety deposit boxes at BNP Paribas' Misrata branch before the war. Most of them are aged between 17 and 20.
Being wounded is one thing, but watching friends die is another.
Groups of weeping young men can often be seen outside Al Hekma hospital in Misrata after one of their comrades has been killed.
At the field hospital closest to the front, one young lightly injured man wandered around shirtless, bandaged and crying inconsolably, not for his own wounds but for the friend who died next to him.
The toll of those losses can be seen in the faces of men like Sofian, a 21-year-old engineering student.
When asked how he adapted to life in the front line, Sofian responds with a laugh "war is fun."
But the laugh does not reach his eyes. When he looks at you, the eyes of an old man stare out of a young face.
"Now we have begun, we have to go all the way to Tripoli," he said. "If Gaddafi wants to get back into Misrata, he will have to come over our dead bodies."
Older men in the city, like Mohammed Erhyam, are impressed.
"We did not expect this from our young people, that they would fight so hard," said the 49-year-old.
"But they are braver than we are. And braver than we thought they would be."
